To Delete that Theme completely, a Tool is necessary, since it is a Custom Theme.
The tool is PS4 Toolset,

USE:
1- Place PS4 Toolset.pkg and Tema_Default.ctmthm on the USB.
2- Install PS4 Toolset.pkg like any homebrew or game.
3- Run PS4 Toolset and go to the "Install Custom Theme" option, look for Theme_Default.ctmthm on the USB and Install to Restore the PS4 to its Original Theme.
4- Uninstall PS4 Toolset and Restart the PS4.
That's it, you can use Normal Themes again.
